Verification Process

Step 1 — Contact Support

Email support@eval.health with your name and email address to initiate the verification process.

Step 2 — Information Collection

During a phone call, you provide personal and professional details including:

  • Full name and contact information
  • Professional credentials and license numbers
  • Specialization
  • Organization name and affiliation

Step 3 — Document Submission

You receive a DocuSign link to submit supporting documentation, which may include:

  • Government-issued identification
  • Professional license
  • Employer verification letter
  • Institutional affiliation proof

Steps 4–6 — Review and Notification

The EVAL team reviews your submission for completeness and accuracy, conducts verbal verification, and cross-checks information against external databases. You are notified of your verified status upon completion.
