Getting started: patient portal

A step-by-step onboarding path for patients — from receiving your invitation to completing evaluations, viewing results, and managing your account.

This guide walks you through your first experience with the EVAL Patient Portal. By the end, you'll know how to log in, complete your assigned evaluations, review your results, and keep your account information up to date.

The Patient Portal is a separate application from the one your healthcare provider uses. It's designed specifically for you — with a simplified interface focused on what you need: completing assessments, checking results, and managing your account.

You don't need to do everything at once. The most important step is completing your first evaluation. Everything else — reviewing results, checking schedules, updating your profile — you can explore at your own pace.

Receive your invitation and create your account

Your healthcare provider will send you an invitation — typically via email — with a link to set up your Patient Portal account. Click the link, create your username and password, and you're in. Your provider has already set up your patient record on their side, so your name and basic information will be pre-filled.

If you receive a task link instead of an account invitation, you can complete a one-time evaluation without creating an account. See Task links for details on how these work.

Sign in and explore the Today page

After signing in, you'll land on the Today page — your home base in the portal. This page shows every evaluation that's due today, organized as cards with the evaluation name, your provider's name, and how many times you need to complete it today.

The left sidebar has four sections: Today (your daily to-do list), Results (your completed evaluations), Schedules (your upcoming assessments), and My Account (your profile and settings). See Today dashboard for the complete overview.

Complete your first evaluation

Click on any evaluation card on the Today page to open it. The evaluation player shows questions one section at a time with a progress bar on the right so you can see how far along you are. Answer each question and click Save when you're done.

After saving, the result is sent to your healthcare provider immediately. On the Today page, the completed evaluation shows a checkmark. If your evaluation has multiple daily slots (for example, "Four Times a Day"), each slot tracks independently. See Completing an evaluation for the full walkthrough.

Review your results

Navigate to Results in the sidebar to see every evaluation you've completed. The Recent tab shows your latest submissions as visual cards, while the All Results tab provides a full list you can scroll through. You can search by evaluation name to find specific assessments quickly.

Click on any result to review exactly what you submitted — your answers are displayed in read-only mode along with any computed scores. This is useful for tracking your progress over time or preparing for an appointment with your provider. Many patients find it helpful to review their recent results before a clinical visit so they can have a more informed conversation with their care team. See Viewing results for details.

Check your schedule

The Schedules page shows all the assessments your providers have set up for you — what evaluations you need to complete, how often, and when the next one is due. Each schedule shows the repeat frequency, next due date, total completions so far, and end condition.

You can't modify schedules yourself (only your healthcare provider can do that), but this page gives you full visibility into your assessment timeline. If you need to change how often you complete an evaluation or when a schedule ends, contact your provider and they can adjust it from their side. See Schedules for more.

Update your profile and account settings

Navigate to My Account to review and update your personal information. You can edit your name, date of birth, email, and phone number — changes sync automatically to your provider's system. You can also upload a profile photo, change your password, and review which healthcare providers have access to your data.

Keeping your contact information current is especially important because it's how EVAL sends you schedule reminders and notifications. If your email or phone number changes, update it here so you don't miss any assessment reminders. See Account and settings for details.

The Patient Portal works on desktop and mobile devices. You can complete evaluations from your phone, tablet, or computer — whichever is most convenient. If your provider sends you email or text reminders, the links in those messages take you directly to the evaluation.
If you receive a provider invitation link you don't recognize, verify with the provider before clicking it. Accepting an invitation grants that organization access to your evaluation data. Only accept invitations from healthcare providers you know and trust.
