EVAL Health employs a quick and simple process where individuals call in to have their accounts verified. Reach out to us at support@eval.health, providing your name and email. We will then call you to start the verification process.

We will follow these steps to verify:

1. Information Collection: We will ask you to provide the following required personal and professional information during the phone call
   • Full Name
   • Email Address
   • Phone Number
   • Professional Title & Credentials
   • Professional License Number
   • Specialization
   • Organization Name
   • Organization Website
2. Document Submission - After the call, we will send you a DocuSign link to send us additional documentation to confirm your identity, such as:
   • A government-issued ID such as a passport, driver's license, or national ID card
   • Copies of professional licenses, certifications, or diplomas
   • A letter or document from the employer verifying the user's position and employment status
   • Documentation proving affiliation with a recognized healthcare or research institution
