How to Become Verified
EVAL Health employs a quick and simple process where individuals call in to have their accounts verified. Reach out to us at support@eval.health, providing your name and email. We will then call you to start the verification process.
We will follow these steps to verify:
- Information Collection: We will ask you to provide the following required personal and professional information during the phone call
- Full Name
- Email Address
- Phone Number
- Professional Title & Credentials
- Professional License Number
- Specialization
- Organization Name
- Organization Website
- Document Submission - After the call, we will send you a DocuSign link to send us additional documentation to confirm your identity, such as:
- A government-issued ID such as a passport, driver's license, or national ID card
- Copies of professional licenses, certifications, or diplomas
- A letter or document from the employer verifying the user's position and employment status
- Documentation proving affiliation with a recognized healthcare or research institution
- Initial Review: Our verification team will review the provided information and documents for completeness and accuracy.
- Phone Verification: We will call you to verbally verify the information and ask additional questions if needed.
- Cross-Verification: The provided information is cross-checked with external databases or relevant authorities to ensure authenticity.
- Confirmation and Notification: Finally, we will notify you of your verified status and your account will be updated accordingly in EVAL.
