Questions
Intro to Questions
After you have created the application shell, you are now ready to add questions to it.
Questions can be set up for numeric entry or multiple choice options. Multiple choice options can be set up as choose one or choose multiple. Every question and choice title should be unique so that you can easily identify them in the data source library.
You will be using low-code or no-code (LCNC) tools to create visibility rules and keywords to automate the workflow logic. You will be able to search a library of questions, choices and results you have created to construct workflows. It is important to be able to differentiate these options for traceability and accuracy.

Navigating to Questions

To start adding questions to your application, you can either:
- Click New Question in the Outline Pane OR
- Click New Question in the Questions tab of the Details Pane
NOTE: After an application is saved and you later go back to the draft, you will see the following items, as shown above, on your application:
- A Publish option to Publish an application (in the green header)
- An Errors tab to Resolve errors
After clicking New Question (from screenshot above), the right pane opens to the following details:

Questions Tab User Interface
Question
Title
Provide your question. This field is required.
Section
Identify the section in which you want your question to display.
All sections must have at least one question. If there are no questions, then delete the section so that an error will not occur.
Input Format
Identify the format of the response. This field is required. Options for this field include:
- Number - requires entry of a numeric value
- One Choice - requires one response to be selected for the question
- Multiple Choices - allows multiple responses to be selected for the question
Navigate to the Applications page for more details on how questions display in Player (by section or one at a time) using the Workflow field.
Description
Optionally provide information for your question. This information will display on the application below the question (in both Builder and Player) to provide further context. You can use various formatting features including: bold, italic, font color, links, bullet list, numbered list, tables, and special characters with the ability to expand the description box to a larger view for editing and un-doing/re-doing changes as needed.
Choices
The Choices tab provides an option to add choices to questions. This section only displays when Input Format is One Choice or Multiple Choices.

Every question must have at least one choice.
Align Vertically
Toggle on/off for how choices are to display. Choices are aligned horizontally by default.
Click New Choice (shown in screenshot above), and the following displays:

Enter a name for the choice
Formula Value
Provide a numerical value for the choice. This is optional. For example, assigning a numeric value to an answer allows you to construct a points-based decision calculator.
Visible When (for Choices)
Set visibility rules around when the choice displays. By default, it will always display. Navigate to the Visibility Rules page to learn more.
Danger Zone (for Choices)
If you no longer want to have this choice in your application, click Delete this Choice and confirm the prompt to delete. This action cannot be undone.
Click Create Choice to save the choice for the question. Repeat the above to add additional choices.
Viewing New Choice(s) in Outline Pane
The Questions tab now reflects the newly added choices as shown below.

Changing Order of Choices
By default, choices will be in the same order as you create them. To change the order, simply click on the 9-dot icon to the right of the choice you want moved (as shown next to Mild, Moderate, and Severe in the image above), and then drag and drop it to the desired location in the list of choices.
Media
You can add images and YouTube videos to the question by clicking New Media. To learn more on adding media, navigate to the Media page.

Number Constraints
This section displays when Input Format is Number.

Whole Numbers Only
Identify if the response to the question will be captured in whole numbers. By default, the value is not a whole number.
Minimum
Optionally identify if the response will have a minimum value.
Maximum
Optionally identify if the response will have a maximum value.
Units of Measure
Adding multiple units of measure gives users running the application a choice on the unit of measure for the entry.
When creating units of measure in Builder, the primary unit of measure will be the default value and the unit of measure used in any computation within the application.
The primary unit of measure will be the default used in formula expressions. EVAL will convert additional units of measure to the primary unit of measure for formula expressions.
For example, a BMI calculator allows the user to enter weight in kg or lbs. Since the primary unit of measure is kg, EVAL will convert the weight entry in lbs to kg when computing BMI.
This section displays when Input Format is Number.

The first unit of measure you enter will be the primary unit
From the Questions tab, click New Unit of Measure as shown above. The following displays:

Unit of Measure
Primary Unit
Provide the measure of the primary unit (e.g. "lb"). This will be the unit that displays, by default, to the user running the application.
Click Create Unit of Measure to save the unit of measure for the question.
The Questions tab now reflects the newly added Unit of Measure as shown below.

Repeat the above to add additional units of measure.
The following shows an example when a second unit of measure is added for kg:

- When adding an additional unit of measure, you will be prompted for the conversion value. After you provide the conversion value, a table will populate below it that shows examples of converting from the primary unit to the secondary unit.
Conversion values must be a valid number that properly converts to the primary unit of measure.
- Verify the conversion values and samples by clicking Verified. This step must be completed before you are allowed to click Create Unit of Measure. The red Patient Safety box will be removed after you complete this step, as shown below:

Click Create Unit of Measure to save changes.
The Details Pane updates as follows:

Danger Zone (for Unit of Measure)
If you no longer want to have this unit of measure in your application, click Delete this Unit and confirm the prompt to delete. This action cannot be undone.
Visible When
Set visibility rules around when the question displays. By default, the first question in the first section will always display. If the application workflow is set up as a decision tree, only the first question will always be visible. The remaining questions will require visibility rules. However, in the calculator workflow, all questions in a section will always appear at the same time unless you change the visibility rules. Navigate to the Visibility Rules page to learn more. A few callouts on setting visibility rules for questions:
The first question in the first section must always be visible and must have at least one visible choice
In a decision tree, questions are only visible when there is at least one visibility rule associated with a question
Danger Zone
If you no longer want to have this question in your application, click Delete this Question.

After clicking Delete this Question, confirm the prompt to delete. This action cannot be undone.
Viewing New Question in Details Pane
After clicking Apply to save changes to your question, the Details Pane will be populated with your new question.

Changing Order of Questions
By default, questions will be in the same order as you create them. To change the order of the questions when you have multiple questions, simply click on the 9-dot icon to the right of the question you want moved (as shown next to Weight in the image above), and then drag and drop it to the desired location in the list of questions.
Viewing New Question in Outline Pane
After clicking Apply to save changes to your question, the Outline Pane will be populated with your new question where you can add additional questions directly from within it or navigate to the Questions tab in the Details Pane and alternatively add there.

Use Cases: Injury Severity Score, MAPPP & BMI Applications
